New Patients

All new patients are expected to arrive at least 30 minutes prior to their scheduled appointment for registration.

 At your first visit you will need to have the following documents completed  prior to seeing the physician: 

1. A filled out copy of your Patient Registration Paperwork (download link below)
2. A drivers license or state issued ID card
3. Your current insurance card(s)
4. A complete list of your current and active medications
5. A complete copy of your medical records (including any labs) if you have them. 
6. Print out and sign both sides of the "Financial Policy & Privacy Practices Acknowledgement Form"

**Male patients need to complete the "Male AUA Survey" 

**Female patients need to complete the "Female Urinary Activity Survey." 

If any of the above documents are missing or incomplete at the time of your initial appointment you will be asked to complete them in their entirety prior to being seen by the physician. If you arrive to your appointment without your current Insurance Card(s) your appointment will be rescheduled.

If you have been seen by a urologist in the past, please advise that office to fax your records to our office in advanced.
